Responsible for collecting, tracking, trending and monitoring data reviewing outcomes, compliance and identifying opportunities for improvement. Uses critical thinking skills to present findings and make recommendations to interdisciplinary health care team. Serves as clinical case manager and conducts new admission assessments as census dictates.
