compassionate and dedicated Family Medicine Physician to join our
outpatient primary care team. This role is responsible for
delivering comprehensive, patient-centered care to a diverse
patient population across all age groups. Key Responsibilities:
Provide preventative, acute, and chronic disease management in a
primary care setting Conduct routine physical exams, diagnose
illnesses, and develop treatment plans Order and interpret
diagnostic tests; prescribe medications and therapies Coordinate
care with specialists, behavioral health, and care management teams
Maintain accurate and timely medical records using an EMR system
Participate in quality improvement initiatives and adhere to
evidence-based guidelines Precept students or residents (if
applicable) Qualifications: MD or DO from an accredited medical
school Board Certified or Board Eligible in Family Medicine Active
California medical license or eligibility Commitment to serving
diverse and underserved populations Excellent communication and
teamwork skills Preferred: Bilingual (English/Spanish) Experience
in community health or value-based care models Interested in
