Per Diem & Part Time Home Health RN – San Diego South County, CA Provide skilled, one-on-one patient care in the home with flexible scheduling and strong clinical support. What You'll Do Perform comprehensive in-home nursing assessments for home health patients Develop and manage individualized plans of care in collaboration with physicians, caregivers, and the interdisciplinary care team Provide skilled nursing services including medication management, wound care, disease management, and patient education Complete OASIS assessments, recertifications, case conferences, and timely clinical documentation Identify the need for additional home health services, disciplines, and durable medical equipment (DME) Assess home environments to promote patient safety, fall prevention, and improved outcomes Obtain and document physician orders, including verbal orders, per agency and regulatory guidelines Educate patients and caregivers on care plans, medications, and symptom management Maintain compliance with CMS, Medicare, and home health regulations Serve as a preceptor or mentor to other nursing staff as needed You’ll Love This Role If… You enjoy one-on-one patient care You want autonomy with strong team support You value flexibility and meaningful work You take pride in delivering skilled, compassionate care Qualifications Graduate of an accredited nursing program Current, active California Registered Nurse (RN) license and CPR certification At least one year of clinical practical nursing experience outside of school or rotations Home Health experience preferred, but not required Valid driver's license, reliable vehicle, and current auto insurance Strong interpersonal and communication skills Ability to make home visits and provide skilled nursing assessments and care Home Health and Home Care Home Base (HCHB) documentation experience preferred, but not required Pay & Perks Competitive pay that reflects your experience and expertise Full-time benefits (23 days of PTO, medical, dental, vision, and disability coverage) available to full-time associates Flexible scheduling that supports work-life balance Mileage reimbursement for patient-related travel Continuing education and tuition assistance Generous referral bonus program OASIS Virtual Assistant to streamline documentation Supportive, accessible leadership team Our Mission & Values Our mission is to deliver exceptional home health care through clinical excellence, compassion, and integrity, while making a meaningful impact in the communities we serve.
